Introduction: Hodgkin's lymphoma (HL) is a rare neoplasm of the lymphatic system, it is one of the most common cancers in the young adult population.
Methodology: Retrospective analysis of 58 patients treated by M. Hodgkin from December 31. 2018. until 31.12.2023. and monitored until December 31. 2025. Including the newly diagnosed HL and excluding relapsing disease. The diagnosis was established by pathohistological analysis of the biopsied lymph gland, immunohistochemistry was performed with the original (DAKO, Gloustrup, Danmark). For the level of expression, the limit value for positivity > 30% . Bulky disease, the size of the tumor mass is greater than 70 mm. Statistical analysis SPS 23.0, significance p< 0.05. The aim of the work: To examine the predictability of the immunohistochemistry record on the size of the tumor mass and the correlation with the IPS score.
Results: Analysis of the association of immunohistochemistry markers with the presence of bulky disease revealed a statistically significant association for CD30 (χ2=4.603; p=0.032), PAX5 (χ2=4.261; p=0.039) and CD3 (χ2=5.695; p=0.017). The marker CD5 showed borderline statistical significance (χ2=3.521; p=0.061), but without sufficient statistical confirmation, while the other markers CD20, MUM1, CD15, KI67, BCL2 and BCL6 did not show a statistically significant association with the bulky disease (p>0.05) at the time of diagnosis. Analysis of the relationship between immunohistochemistry markers and the diameter of the tumor mass revealed a statistically significant for CD5 (χ2=5.839; p=0.016) and CD15 (χ2=4.766; p=0.029). In patients with CD5-, the maximum diameter of the tumor mass was 37 mm, and in CD5+ it was 98 mm. An inverse association was observed for CD15, with CD15- the maximum diameter was 98 mm, and CD15+ was 43.5 mm. The other markers have no statistically significant association with the diameter of the tumor mass (p>0.05). Analysis of the relationship between immunohistochemistry markers and the IPS score a statistically significant association was observed only for CD3 (χ2=4.016; p=0.045). All patients with positive CD3 expression had a low IPS score (100%). Other markers did not show association with IPS score (p>0.05).
Conclusion: Immunohistochemistry markers in the studied show selective significance. CD30, PAX5 and CD3 showed a correlation with the size of the tumor mass and they can be a predictor of tumor burden. Only CD 3 shows a correlation with IPS score.
